Please find the Application Documents for 2012 April below.　　

Please download and print out all of the documents below in order to apply for the school year of 2012. Please note that the school will only aim to accept documents sent by post between November 1st and 11th (during this first round application period).

2012年度4月入学・入園・編入希望者の出願書類一式（2〰4の計3種類）を以下からダウンロード、プリントアウトしてお使いください。　出願期間は11月1日（火）〰11月11日(金）となり、出願書類の受付は郵送のみとなります。

※If you are living in Japan, and would like to apply, please ask the school to send the application documents by post as the application fee should be paid using the designated bank transfer form which is not available on the website. If you are living abroad, this bank transfer form is not needed when paying the application fee. Please refer to the Admission Booklet for details (page 3). If you would like to have the documents sent by post, please contact the school at info@mis.ed.jp

国内にご在住で出願を検討されている場合には、出願料の振込用紙（ホームページには掲載しておりません）が必要ですので、学校から出願書類を取り寄せてください。　海外にご在住の場合には、出願料を支払う際の振込用紙は不要です。　詳細については、募集要項(3ページ）をご参照ください。　出願書類の取り寄せを希望される場合は、お問い合わせください
